Hospice Social Worker

We are seeking a skilled and compassionate Social Worker to join our team at TN Quality Care Hospice in Waynesboro, TN. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of patients and families by providing psychosocial support and counseling services.

Estimated Salary: $70,000 - $90,000 per year

The ideal candidate will have a strong background in social work, excellent communication skills, and the ability to work effectively in a fast-paced environment. You will be responsible for performing thorough assessments of patients and families, developing individualized plans of care, and providing counseling and support to patients and families.

Key Responsibilities

  • Perform thorough assessments of patients and families to identify their needs and develop individualized plans of care.
  • Provide counseling and support to patients and families to help them cope with stress and other challenges related to illness or caregiving responsibilities.
  • Collaborate with healthcare teams to ensure comprehensive care and address patient needs.
  • Document patient interactions and progress in a timely and accurate manner.
  • Develop and maintain relationships with community resources to access additional support services for patients and families.

Requirements

To be successful in this role, you will need:

  • A Master's degree in Social Work (MSW) from an accredited institution.
  • Licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
  • At least one year of experience working in healthcare or a related field.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
