Role Summary:

We are seeking a compassionate End of Life Care Specialist to join our team in Waynesboro, TN.

Job Description:

This is a full-time position that offers immediate openings and generous benefits. You will be responsible for providing psychosocial assessments of patients and families/caregivers, counseling them as needed, and assisting with applications for financial assistance. Your exceptional service skills and ability to work effectively in a team environment will make you a valuable asset to our organization.

Responsibilities:
  • Conduct thorough assessments of patients' needs and develop individualized plans of care
  • Counsel patients and their families on coping with stress and other challenges related to end-of-life care
  • Assist with applications for Medicaid, Supplemental Security Income, and other forms of financial assistance
  • Liaise with community agencies to ensure patients receive necessary resources
Requirements:
  • A Master's Degree in Social Work from an accredited institution
  • Current State Licensure in Social Work (LMSW or LCSW)
  • At least one year of experience working in healthcare or hospice settings
  • A valid driver's license and proof of insurance
