Job Description

We are seeking a compassionate and skilled Hospice Social Worker to join our team. As a key member of our interdisciplinary team, you will provide high-quality psychosocial support to patients and their families/caregivers in the hospice program.

Key Responsibilities
  • Perform timely psychosocial assessments of patients and families/caregivers, ensuring identified needs are addressed in the patient's plan of care.
  • Counsel patients, families, and staff as needed to address stress and coping difficulties.
  • Provide accurate and timely documentation of patient services, reflecting the plan of care.
  • Assist with applications for social security benefits, hospice, Medicaid, and Medicaid waiver programs.
  • Serve as a liaison between patients, families, and community agencies.
Required Skills and Qualifications

To be successful in this role, you will need:

  • A Master's Degree in Social Work (MSW) from an accredited institution.
  • Current State Licensure in Social Work (LMSW or LCSW).
  • A minimum of one year's experience in health care or hospice settings.
  • A valid driver's license and proof of insurance.
