The Road Home has dedicated itself to combating homelessness for a century. We offer accessible emergency shelters, comprehensive support services, and housing-first rental assistance aimed at helping individuals and families transition from homelessness to stability within the community.
The Permanent Supportive Housing Case Manager will join our Housing Case Management Team, delivering direct case management services to families who have successfully transitioned from homelessness and are now residing in various housing situations across the region. This role involves connecting families with essential resources and collaborating closely with the Case Management Team, partner agencies, and community organizations to address the diverse needs of these families. The case management responsibilities will include office work, home visits, and transportation of clients throughout the area, all while emphasizing Trauma-Informed Care and Housing First principles.

Reports To: Permanent Supportive Housing Supervisor
Work Schedule: Monday - Friday, 40 hours per week
Salary Grade: Grade 6, $18.90/hour
FLSA Status: Non-Exempt

- Assist families and individuals with long-term rental assistance vouchers in stabilizing their housing and community integration.
- Utilize a housing-focused and strengths-based approach tailored to meet the unique needs of each tenant.
- Maintain familiarity with rental assistance funding sources.
- Implement Trauma-Informed Care and Housing First practices in service delivery.
- Provide clients with referrals to programs and connect them to community resources, facilitating access to these services.
- Deliver direct case management, including crisis intervention, ongoing assessments, goal setting, and problem-solving.
- Support clients in securing employment and applying for Social Security benefits.
- Advocate for clients with property managers and negotiate on their behalf.
- Evaluate children's needs to make suitable community referrals and coordinate with schools for supportive services.
- Assist tenants in applying for waitlists for Section 8, Public, and Subsidized Housing.
- Participate in weekly staff meetings, case discussions, and additional meetings as required.
- Engage in daily collaboration with other departments, fostering a positive team environment.
- Maintain accurate records, case notes, and statistical data as assigned.
- Perform other duties as assigned.

Education and Experience:
- Bachelor's Degree in a relevant field is preferred.
- Experience working with diverse populations is preferred.
Skills and Requirements:
- Ability to work independently and collaboratively within a team.
- Capacity to manage high-stress situations effectively.
- Strong interpersonal skills with the ability to engage with diverse populations.
- Excellent organizational abilities.
- Proficient in computer skills, including Microsoft Office applications.
- Access to reliable transportation; personal vehicle is advantageous.
- Must pass a pre-employment background check and drug screening.
- Must have a clean driving record and be insurable under agency policy; driving is a daily requirement.
Physical and Equipment Requirements:
- Ability to lift up to 25 pounds.
- Ability to stand and/or walk for extended periods.
- Willingness to drive an agency vehicle.
