Job Summary

We are seeking a Certified Nursing Assistant (CNA) to join our Homecare team. As a CNA, you will work closely with our interdisciplinary team to provide quality care to patients and their families in their homes.

Key Responsibilities
  • Assist patients with activities of daily living, such as bathing, dressing, and grooming
  • Provide personal care, including toileting, incontinence care, and ambulation
  • Assist with eating and drinking, and perform post-mortem care as needed
  • Obtain vital signs and assist with treatments
  • Anticipate and respond to patient needs, and provide homemaker assistance as needed
  • Perform light housekeeping, such as changing linens and tidying rooms
  • Order and deliver patient supplies, and help prepare simple meals
  • Communicate effectively with patients, families, and healthcare team members
  • Document patient care and changes in condition in electronic health records
Requirements
  • High school diploma or equivalent
  • Graduation from an accredited Certified Nursing Assistant program
  • Current, unrestricted Nursing Assistant Certification in the state of employment
  • One year of personal care experience, hospice, or home health experience preferred
  • Valid driver's license, auto insurance, and access to a reliable vehicle
  • Ability to develop, maintain, and apply knowledge of hospice and end-of-life care
  • Ability to maintain professional boundaries with patients, families, and coworkers
  • Ability to create and follow plans of care, and patient, family, and clinical staff directions
  • Basic Microsoft Office skills
  • Experience using electronic health records; Epic preferred
