Clinical Specialist II Job Description

Cambridge Health Alliance (CHA) is seeking a full-time Clinical Specialist II, Nurse Practitioner, to join our CHA PACE Program. CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). Our team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.

Key Responsibilities:
  • Assess the physical and psychosocial health of individuals and families by obtaining health, medical, and social histories using the problem-oriented (SOAP) recording system.
  • Evaluate, interpret, and record the developmental status of the patient and family to plan and execute proper intervention.
  • Perform physical examinations, diagnose health and development problems, and care for patients suffering from acute and chronic diseases.
  • Initiate, evaluate, and modify the effectiveness of therapeutic plans, collaborating with other members of the healthcare team.
  • Order laboratory and radiographic studies as necessary, evaluate results, and determine which findings require consultation or referrals to other providers.
  • Work with the community to assess health needs, assist in the development and implementation of programs designed to meet community needs, and maintain accurate records.
Requirements:
  • Minimum of 2 years of experience as a Nurse Practitioner.
  • Graduate of an accredited Nurse Practitioner program.
  • Current Massachusetts licensure authorizing practice in an expanded role, DEA number, and Mass DPH registration to prescribe controlled substances.
