Clinical Specialist II Job Description

Cambridge Health Alliance (CHA) is seeking a skilled Clinical Specialist II to join our CHA PACE Program. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality healthcare services to adults 55 years and older with chronic medical and/or mental health difficulties.

Key Responsibilities:
  • Assess the physical and psychosocial health of individuals and families by obtaining health, medical, and social histories.
  • Evaluate, interpret, and record the developmental status of patients and families to plan and execute proper interventions.
  • Perform physical examinations, diagnose health and development problems, and care for patients suffering from acute and chronic diseases.
  • Initiate, evaluate, and modify therapeutic plans, collaborating with other healthcare team members as needed.
  • Order laboratory and radiographic studies, evaluate results, and determine which findings require consultation or referrals to other providers.
  • Work with the community to assess health needs, develop, and implement programs to meet those needs.
  • Maintain accurate records, referral books, and tickler files for follow-up of specific problems.
  • Participate in quality care evaluations, including record audits and initiation/revision of guidelines, policies, and procedures.
Requirements:
  • Minimum of 2 years of experience as a Nurse Practitioner.
  • Graduate of an accredited Nurse Practitioner program.
  • Current Massachusetts licensure authorizing practice in an expanded role, DEA number, and Mass DPH registration to prescribe controlled substances.
