Key Responsibilities:- Coordinate healthcare services and support for clients, working closely with medical providers and other service providers.
- Provide advocacy and support during medical appointments, ensuring clients receive the best possible care.
- Act as a liaison between clients, families, and medical providers, facilitating effective communication and care coordination.
- Reconcile medication with providers and clients, ensuring accurate and timely medication management.
- Monitor outcomes of appointments and encourage completion of necessary medical follow-up.
- Collaborate with social work to address psycho-social needs and services.
- Provide home visits to educate clients on healthy lifestyle choices and provide support as needed.
- Maintain accurate and timely documentation of client interactions and care coordination.
- LPN License and New York State Certification required.
- One year of experience in the healthcare field preferred but not required.
- Knowledge and experience with geriatrics, intellectual and developmental disabilities preferred.
- Ability to conduct home visits and work independently with a great attention to detail.
- Excellent communication and listening skills, with a focus on treating clients with respect and dignity.
Compensation: $68,000 - $72,800 commensurate with experience.
