We are seeking licensed mental health professionals to join our Care Navigation team (LCSW, LMFT, LPC required) to assist members dealing with posttraumatic stress disorder (PTSD) and Complex Trauma. The Trauma Clinical Care Navigator will conduct initial assessments, provide support, facilitate referrals, and ensure ongoing care coordination for participants in Spring Health's innovative Trauma program.

Key Responsibilities:

  • Conduct telephonic screenings, brief interventions, and referrals to trauma specialty treatment providers.
  • Employ active listening, psycho-education, and motivational interviewing techniques to understand each member's needs and support them in goal-setting.
  • Implement Judith Herman's phase-based trauma treatment model to offer stabilization support to members engaging in care.
  • Utilize Motivational Interviewing to encourage members to participate in their care.
  • Assess appropriate levels of care, facilitate referrals, and coordinate care with Spring Health providers.
  • Provide consultations regarding mental health concerns and family/relationship issues as needed.
  • Offer immediate support and safety planning for members facing risks or crises.
  • Communicate effectively with members and providers through written correspondence.
  • Maintain accurate records, document information, perform follow-ups, and track progress within various systems, including an EHR.
  • Identify and address barriers or disruptions to care.
  • Collaborate with multi-disciplinary teams such as care support, provider operations, and benefits/billing to ensure a seamless member experience.
  • Engage in staff meetings, case consultations, and training sessions.
  • Assist in onboarding and mentoring new team members.
  • Adhere to Spring Health policies while maintaining confidentiality, compliance, and ethical standards.

Qualifications:

  • Required mental health license: LCSW, LMFT, LPC.
  • Minimum of 2+ years of clinical experience working with PTSD and Complex Trauma.
  • Familiarity with motivational interviewing, diagnostic assessments, and triage.
  • Knowledge of phase-based trauma treatment models and evidence-based practices for PTSD (e.g., Prolonged Exposure, Cognitive Processing Therapy, EMDR, ART).
  • Strong listening, written, and verbal communication skills.
  • Commitment to diversity and inclusion.

  • Dedicated to providing a high-touch experience for members.
  • Proficient in technology and comfortable using various computer platforms.
  • Able to manage multiple tasks and switch contexts effectively.
  • Performance metrics will include connection to care, care conversion, clinical outcomes, and member satisfaction.

Preferred Qualifications:

  • Trauma-specific licensure or certifications.
  • Bilingual abilities.

Compensation and Benefits:

The target base salary for this role is $42.31 per hour, complemented by a competitive total rewards package that includes stock options and benefits. Individual compensation may vary based on experience, location, and other relevant factors.
