Job Summary

We are seeking a highly skilled Patient Navigator to join our team at the University of Miami Health System. As a Patient Navigator, you will play a critical role in facilitating patients' access to healthcare services by identifying barriers to care, coordinating treatment plans, and monitoring the implementation and follow-up of services.

Key Responsibilities
  • Conduct Patient Evaluations: Conduct thorough patient evaluations to gather information necessary for developing effective treatment plans.
  • Ensure Access to Insurance and Social Services: Verify patients' eligibility for insurance and social service programs, and ensure they have access to these resources.
  • Provide Patient Reminders and Liaison Services: Remind patients of upcoming appointments and serve as a liaison between patients and healthcare providers.
  • Identify and Resolve Barriers to Care: Identify and address real or perceived barriers to care, ensuring patients receive the necessary support and resources.
  • Provide Education and Counseling: Educate and counsel patients on health-related issues and life situations, promoting informed decision-making and self-advocacy.
  • Stay Current on Community Resources: Maintain knowledge of community health resources, welfare programs, and other services available to patients.
  • Adhere to University Policies and Procedures: Comply with University and unit-level policies and procedures, safeguarding University assets and maintaining confidentiality.
Department-Specific Functions
  • Respond to Families of Children/Adults with Disabilities: Provide support and resources to families of children/adults with disabilities, responding to inquiries and referrals.
  • Assist Families in Identifying Strengths and Needs: Help families identify their strengths and needs, providing guidance and support.
  • Link Families to Disability Services: Connect families with disability services, utilizing community databases, expertise, and research.
  • Provide Support, Information, and Referral Advice: Offer support, information, and referral advice to families, linking them to community support and disability services.
  • Follow Up with Families: Follow up with families to ensure they receive necessary disability services and additional supports.
  • Maintain Accurate Records: Maintain accurate and timely records in the program database.
  • Attend Trainings and Meetings: Attend trainings and meetings as needed, staying current on best practices and policies.
  • Organize and Prioritize Tasks: Organize, prioritize, and establish schedules to accomplish program goals and evaluate the delivery of services.
  • Experience Navigating Disability Services: Possess experience navigating federal, state, and local disability services, with a strong understanding of relevant policies and procedures.
Qualifications
  • High School Diploma or Equivalent: Possess a high school diploma or equivalent, with a minimum of 3 years of relevant work experience.

  • Bilingual Proficiency: Possess bilingual proficiency in English/Spanish, with the ability to communicate effectively with patients and families.

  • Effective Communication Skills: Possess the ability to communicate effectively in both oral and written form.
  • Professional Composure: Demonstrate the ability to handle difficult and stressful situations with professional composure.
  • Interpersonal Relationships: Possess the ability to maintain effective interpersonal relationships with patients, families, and healthcare providers.
  • Instruction Following: Demonstrate the ability to understand and follow instructions, with a strong attention to detail.
