Job Summary:

The RN Referral Coordinator plays a critical role in facilitating the referral process for patients requiring skilled, hospice, and private duty home care services. This position is responsible for obtaining and processing referral information, coordinating patient care services, and ensuring seamless communication between healthcare providers.

Key Responsibilities:

  • Facilitate communication and coordination of patient care services between hospitals, rehabilitation centers, sub-acute units, physician offices, and home care agencies.
  • Receive and process program-specific referral information, adhering to department protocols and agency procedures.
  • Meet with patients in facilities to coordinate needed home services, DME, infusion supplies, and other requirements.
  • Review community referral forms for accuracy and clarify ordered services as needed.
  • Assess home safety and environmental concerns prior to discharge from facilities and identify POA if known.
  • Receive and process orders for home care from physician offices and other outpatient sources, assessing feasibility and discussing concerns with physicians.
  • Enter referral information into the Horizon Homecare system, creating and signing initial interim orders for home care services.
  • Collaborate with other home care agencies to route referral information and other data in a timely manner.
  • Follow agency protocol for patient assignment within the home care geographic matrix.
  • Work with support staff to manage job duties effectively within time frames.
  • Establish primary physician prior to referral acceptance and collaborate with support staff to verify diagnosis coding, insurance information, and benefits.
  • Demonstrate commitment to customer service, introducing self to customers, eliciting information, and responding to inquiries.
  • Demonstrate organizational skills to effectively manage caseload, including high acuity/complex cases.
  • Pursue educational opportunities to enhance competencies, knowledge base, and skills, participating as a preceptor/mentor for students and new employees, networking with other professionals, and attending professional in-services.
  • Participate in unit meetings, task force activities, and quality improvement studies as requested.
  • Perform other duties, including home visits, as directed.

Requirements:

  • Licensed to practice professional nursing in the states of Ohio and Michigan.
  • Minimum of 1 year recent acute care clinical/home care experience.
  • Current CPR certification.
  • Excellent organization and problem-solving abilities.
  • Strong customer and team orientation.
  • Ability to work with computerized documentation systems.
