Job Summary:

We are seeking a compassionate Medical Care Specialist to join our team at Abode Care Partners. As a skilled nurse practitioner, you will provide evidence-based medical care to seniors and patients with complex needs in various settings.

Responsibilities:
  • Perform comprehensive medical assessments and develop patient-centered care plans
  • Diagnose and treat chronic and acute illnesses, minor injuries, and other health conditions
  • Collaborate with healthcare professionals to ensure interdisciplinary care and maintain accurate patient records
  • Order and interpret laboratory and diagnostic testing as needed
Requirements:
  • Active state Nurse Practitioner license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
Salary and Benefits:

The estimated annual salary for this position is between $60,000 and $72,000, depending on qualifications and experience.
