Job Overview

We are seeking a dedicated Clinical Support Specialist - Community Care Coordinator to join our dynamic team

Key Responsibilities:
  • Connect families with needed community resources.
  • Complete documentation within established time frames.
  • Meet with clients weekly or bi-weekly for up to 12 months.
  • Attend and participate in Treatment Team meetings.
Requirements:

Bachelor's degree in human services discipline, including Social Work, Psychology, Sociology, Counseling, Criminal Justice, etc. and experience working with children and families in case management type/community resource position.

Salary and Benefits:

The estimated annual salary for this role is $60,000-$80,000 based on location and industry standards. We offer a comprehensive benefits package, including medical, dental, vision, and other voluntary insurance products.
