Crisis Case Manager Job Description

AHRC New York City is seeking a skilled Crisis Case Manager to join our Crisis Intervention team. As a Crisis Case Manager, you will work collaboratively with team members to provide emotional, social, vocational, and housing support to individuals in crisis.

Key Responsibilities:
  • Collaborate with Crisis Intervention team members to identify and address the needs of individuals in crisis.
  • Provide referral and case coordination services to support individuals in achieving stability.
  • Utilize community resources to assist individuals in crisis.
  • Develop and implement service plans to address the needs of individuals in crisis.
  • Conduct supportive services, including in-person, in-home, and teletherapy sessions.
Requirements:
  • Bachelor's degree in a Human Services discipline from an accredited university program.
  • Work experience with people with developmental disabilities.
  • Strong interpersonal, written, and verbal communication skills.
  • Excellent skills in using MS Office applications and agency-specific database systems.
  • Ability to work productively in a collaborative setting and coordinate needs of multiple stakeholders.
