About the Role:

Job Summary

Accountable for providing patient care in alignment with established standards of practice across various environments, including home care, hospitals, long-term care facilities, assisted living, and retirement communities within the hospice service area. Works collaboratively with other healthcare professionals and members of the hospice interdisciplinary team to ensure effective and efficient patient care delivery, aiming for optimal patient outcomes. Integrates knowledge of the patient's age, gender, and cultural background into the care provided. Engages in quality improvement initiatives that demonstrate positive patient care results. Oversees the work of Hospice aides and provides on-call support for the hospice program.

Minimum Job Qualifications

  • Licensure or other certifications: Valid RN licensure in Georgia. Current BLS certification.
  • Educational Requirements: Associate's Degree in Nursing.
  • Minimum Experience: At least one (1) year of clinical nursing experience.
  • Other:

Preferred Job Qualifications

  • Preferred Licensure or other certifications: CHPN certification.
  • Preferred Educational Requirements:
  • Preferred Experience: Experience in caring for terminally ill patients, adults in acute care settings, and home care. Additional experience with pediatric patients is a plus. One (1) year of hospice experience is preferred.

Essential Knowledge, Skills, and Abilities

  • Strong clinical assessment and nursing skills.
  • Ability to interpret and comply with regulations, agency policies, and accreditation standards.
  • Exceptional customer service and interpersonal abilities.
  • Proficient in documenting clinical care using a computerized system.
  • Able to work independently and collaboratively within a team.
  • Frequent travel throughout the service area; must possess a valid driver's license and current auto insurance with a minimum of $100,000 liability. Availability for 24-hour on-call assignments as scheduled.

Key Responsibilities

  • Conducts patient admissions to the hospice program, assessing eligibility based on regulatory requirements and hospice criteria in collaboration with the attending physician and hospice medical director. Completes admission visits within designated timeframes.
  • Delivers nursing care according to the standards set forth by the American Nurses Association (ANA) and the Standards of Hospice Nursing Practice. Performs routine, admission, death pronouncement, continuous care, and on-call visits as required. Implements care plans and evaluates patient responses to interventions.
  • Documents patient care in compliance with medical documentation standards and agency policies, ensuring timely and accurate charting.
  • Collaborates with patients, families, and the interdisciplinary team to ensure coordinated care and effective case management. Respects patient autonomy and dignity while addressing their needs and goals.
On-Call Responsibilities

  • Provides after-hours on-call coverage, responding to patient and family needs with professionalism and compassion. Ensures timely visits when necessary and demonstrates sound judgment in decision-making.
  • Participates in quality improvement initiatives and peer reviews as directed, documenting any variances or incidents according to agency policies.
  • Engages in ongoing professional development, attending team meetings and educational programs to maintain clinical expertise and compliance with regulatory standards.
