About the Role:

Job Summary

Under the general supervision of the Practice Manager and HPB Surgeons, the HPB Program Coordinator, a Registered Nurse, plays a crucial role in guiding patients through the healthcare continuum during their engagement with the NGPG practice and while under the care of an NGPG Hepato-Pancreato-Biliary Surgeon or Physician Assistant. Key responsibilities include the planning, execution, coordination, monitoring, and assessment of personalized care options, services, resources, education, and follow-up requirements. The coordinator manages access to urgent and semi-urgent patient referrals to and from the practice and other ancillary programs within NGHS and surrounding markets involved in the HPB patient's care. This role serves as the primary clinical contact and is responsible for HP data management, including the creation of scheduled and requested reports. The coordinator evaluates ongoing system processes and develops quality improvement initiatives to enhance clinical care and resource allocation.

Essential Tasks And Responsibilities

  • Builds strong relationships with key referrers to enhance access and case volume. Primarily responsible for utilizing the nursing process to address the healthcare needs of assigned patients within established standards of care.
  • Acts as the initial point of contact for patients, their families/representatives, external agencies, physicians, and referrers. Responds to inquiries for access to Hepato-Pancreato-Biliary care, typically received through Epic work queues and Online Inquiry.
  • Coordinates outpatient referrals and, when necessary, manages admission/scheduling for appropriate inpatient needs for NGPG General Surgery and Hepato-Pancreato-Biliary services.
  • Proactively markets NGPG Hepato-Pancreato-Biliary treatment options to internal clinics and external groups through direct education to clinic staff, health fairs, community outreach fairs, and/or community events.
  • Maintains and develops collaborative relationships with referral coordinators, both internal and external, to ensure no barriers or delays in scheduling consultations to the clinic.
  • Evaluates and enhances practices for all clinical care of the Hepato-Pancreato-Biliary program. Serves in a primary nursing role working closely with the surgeon and PA during clinic as well as non-clinic days as needed.
  • Collaborates with physician leadership and designees (i.e., resident physicians) in Hepato-Pancreato-Biliary Surgery to review and develop quality improvement processes within the program. Coordinates with NGHS and NGMC leadership as needed for quality improvement within the scope of the HPB program.
  • Responsible for developing and assessing data reports from the HPB-NSQIP database system.
  • Works closely with the surgery scheduler to optimize all surgical scheduling and patient management, and is able to function as surgery scheduler as needed.
  • Coordinates with administration of NGHS, NGPG, and Cancer Center in program development and quality improvement.
  • Identifies trends with high-volume referring offices and collaborates with Business Development to continually foster positive community referral relationships.
  • Acts as a backup to maintain QGenda schedules for Surgeon and Resident Call Schedules as requested.
  • Identifies and enters IT ServiceNow Tickets, reports variances using RL6 system software.
  • Contacts potential patients for Histotripsy Treatment and is responsible for creating charts, requesting records, collecting and maintaining data, and ensuring initial patient contact for candidacy for the Histotripsy program.
  • Functions as an expert Clinician and mentor for staff.
  • Other duties as assigned.
