Position Overview:

The Physical Therapist is accountable for assessing and managing all patients referred to the skilled nursing unit, swing bed, and outpatient environments. The primary objective is to deliver therapeutic services that aid in restoring function, enhancing mobility, alleviating discomfort, and preventing or minimizing lasting physical impairments resulting from injuries or illnesses.

Effective communication and collaboration with physicians, referring providers, nursing personnel, and other healthcare professionals are essential to ensure the delivery of high-quality care.


Qualifications and Requirements:

Licensure and Certifications:
Must possess a current North Dakota License and fulfill the requirement of 25 Continuing Education Units every two years to maintain licensure.

Educational Background:
Graduate from an accredited Physical Therapy Program.
Proficiency in speaking, reading, writing, and understanding English is mandatory.

Experience:
No prior experience is required; however, experience in physical therapy or the healthcare sector is advantageous.

Special Skills and Training:
A positive disposition towards the elderly and ill
Basic computer proficiency
Willingness to provide PT services in aquatic settings as necessary
Encouraged to join the APTA for professional growth and CEU discount eligibility for the facility.

Core Responsibilities:

  • Ensure the safety of patients, residents, visitors, and staff while executing all duties and responsibilities.
  • Conduct evaluations for Physical Therapy on referred patients, utilizing therapeutic exercises, neurological-developmental techniques, and modalities to enhance posture, gait, mobility, strength, endurance, balance, joint mobility, flexibility, circulation, and developmentally appropriate motor skills, while also alleviating pain and slowing or halting the progression of physical disabilities.


Document a comprehensive summary of the Physical Therapy evaluation, including assessment, treatment plan, frequency, and measurable goals.


  • Responsible for recording Physical Therapy interventions for all patients using the SOAP format (subjective, objective, assessment, plan).
  • Assist with the restorative therapy program for nursing home residents.
  • Conduct Physical Therapy evaluations for nursing home admissions and determine the necessity for formal PT, Restorative, or Walk to Dine programs, communicating findings with nursing staff and PTAs.
  • Maintain confidentiality in compliance with HIPAA regulations when not present in the PT department.
  • Provide clinical instruction and supervision for PT or PTA students and contribute to their evaluations.
  • Assist with patient scheduling and treatment documentation, including correspondence with insurance and referring providers.
  • Ensure the Physical Therapy department remains clean and organized.
  • Order necessary Physical Therapy supplies from Purchasing as required.
  • Complete letters of Medicare non-coverage for patients receiving physical therapy.
  • Adhere to Residents' Rights regulations.
  • Participate in orientation, training, mandatory education, in-services, staff meetings, and educational courses to enhance knowledge and skills.
  • Maintain confidentiality of Resident and Patient information.
  • Treat all individuals with kindness, dignity, and respect.
  • Encourage teamwork and motivate others to perform to the best of their abilities.
  • Exhibit a positive attitude and adaptability to change, while adhering to facility rules and regulations.
  • Comply with the company dress code.
