The Community Engagement Coordinator is responsible for delivering comprehensive, community-based support services that involve both the child and their parent or caregiver.

Service Delivery Responsibilities

• Employ a motivational, stage-appropriate methodology to provide tailored services based on the strengths, needs, abilities, and preferences identified collaboratively with the client and their family, aimed at enhancing the client's functioning and achieving their therapeutic objectives.

• Collaborate with parents to ensure their active participation in the treatment process, which is essential for achieving better clinical outcomes.

• Deliver services to both the child and parent/caregiver utilizing a System of Care approach, emphasizing a holistic, person-centered, strengths-based perspective, while incorporating coaching on the Eight Dimensions of Wellness, focusing on all aspects of wellness, including both physical and mental health.

• Assist clients and families in accomplishing the goals outlined in their Wellness Plans, regularly reviewing these plans with them, reassessing their needs, available service options, community engagement, and updating goals and objectives as necessary.

• Support families in locating, accessing, and coordinating services and resources to help them achieve their objectives.

• Maintain communication with clients and families during hospitalizations, participating in discharge planning, and collaborating with all involved providers to develop aftercare plans.

• Provide educational resources and skill-building opportunities to clients and families regarding daily living skills, coping strategies, education, symptom management, and communication/relationships, with a focus on enhancing overall functioning.

• Develop and implement crisis intervention strategies with families and provide necessary crisis intervention services, including after-hours support.

• Assist clients and families in building natural support networks, working with family members, educational staff, legal guardians, and others.

• Engage in ongoing collaboration with the child and family interagency team members to advocate for the mental health needs of the child and family.

• Complete documentation collaboratively with clients and families, ensuring all required documentation is submitted within the designated timeframe, in accordance with agency policies.

• Meet service delivery expectations based on client needs and agency guidelines. Services are provided in the home environment, accommodating clinical needs, including evenings and weekends.

• Perform additional duties as assigned.

Integrated Health Collaboration

• Actively communicate with other members of the integrated health team, including psychiatrists, therapists, primary care providers, specialists, nurses, and others regarding all aspects of client wellness, including medication adherence, side effects, and effectiveness.


• Seek and incorporate feedback from integrated health team providers into the client's Wellness Plan.

• Make referrals to additional services or providers as necessary for the client's integrated health team.

• Function as a liaison to all parties involved in the client's treatment, including family, educational institutions, and legal guardians.

Additional Responsibilities

• Strive to identify culturally competent services and providers to meet the unique cultural needs of each client and family served.

• Note that driving is a fundamental aspect of this position.
