Job Title: In-Home Program Coordinator

At Support Services of Virginia, we are committed to making a positive and lasting impact on everyone we encounter. We are seeking a dynamic and experienced professional to join our team as the In-Home Program Coordinator.

Job Summary:

The In-Home Program Coordinator will play a vital and mission-critical role overseeing In-Home services and support for people with Developmental Disabilities in the Lynchburg area. This role is the contact point for all Site Coordinators and Direct Support Professionals to ensure smooth daily operations and collaboration.

Responsibilities:
  • Perform all duties with Support Services of Virginia's mission, vision, and core values as guideposts.
  • Create an inspiring team environment within the In-Home program with a culture of clear and effective communication.
  • Set clear team goals and motivate team members to work towards them.
  • Discover skills development needs of team members and provide guidance and coaching.
  • Delegate tasks based on team members strengths and skills, set deadlines, and follow up.
  • Listen to team members' feedback and resolve any issues or conflicts.
  • Oversee day-to-day operation of regional In Home services interfacing with team members across programs and departments.
  • Monitor team performance and report on metrics regarding such items as scheduling and staffing, resource allocation and expenses, employee performance and competencies, serious incidents, and risk mitigation.
  • Point of contact for and tracking of referrals to programs.
  • Works closely with internal and external clinicians to provide input for and feedback on Person Centered Individual Support Plans and other plans of care.
  • Facilitates, attends, and documents team, site, and individual meetings to enhance communication and ensure consistency of services and skills.
  • Provide on-call support.
Requirements:
  • Proven work experience as a leader and working with people with Developmental Disabilities.
  • Extensive understanding of DMAS (Department of Medical Assistance Services), HCBS (Home & Community Based Services), DBHDS (Department of Behavioral Health and Developmental Services) & Licensure regulations.
  • Qualified Intellectual / Developmental Disability Professional Certification (or eligible for certification).
  • Excellent communication and motivational skills.
  • Possess compassion, honesty, independence, and integrity.
  • Ability to inspire others to do their best and to work well with others.
  • Can create and maintain a positive environment, even when dealing with challenging people and situations.
  • Ability to combine and process information and create clear, concise, and timely reports and feedback.
  • Flexible with weekly schedule to meet programmatic needs when they arise and attend required meetings and training on and off site.
  • Computer skills (Microsoft Platforms and experience using training, payroll, and electronic health records systems).
  • Extensive understanding of Developmental Disabilities.
  • Must be fully vaccinated against COVID.
  • Ability to pass criminal/registry background checks.
  • Valid Virginia Driver's License.

  • Bachelor's Degree in a Human Services Field.
