Rehabilitation Program Coordinator

We are seeking a highly skilled and experienced Rehabilitation Program Coordinator to join our team at Centra Health. The successful candidate will be responsible for coordinating, monitoring, and submitting necessary documentation required by the State and Federal Government, facilitating IDT care plan meetings, and working closely with commercial payers for pre-certifications and case management.
Key Responsibilities- Coordinate and monitor PPS forms for accuracy and completion within designated time frames
- Responsible for clinical data collection, entry, transmission, and analysis to ensure compliance
- Act as a resource person for nursing and therapy staff in IDT meetings and ensure an appropriate plan of care for each patient
- Maintain confidentiality of all resident information and respect the rights, privacy, and dignity of each patient
- Maintain clinical competencies by utilizing the nursing process or frame of reference for the delegation and coordination of patient care within the scope of nursing practice
- Ensure electronic submission of assessments to state per schedule and work with MIS to update software
- Act as a resource person for nursing and other disciplines to ensure compliance in data collection and completion of care plans
- Coordinate data input with coding office, billing office, and admissions to ensure validity
- Stay up-to-date with all Federal Government changes related to PPS and incorporate regulatory mandates into routine practices
- Identify quality assessment/data problems and initiate and implement outcome measures
- Understand other certifying agency standards such as JCAHO and CARF and ensure that data is accurate and complete to meet specific requirements
- Work with Social Worker, AR Director, and Medical Director to coordinate IDT Care Plan Meetings with prescribed time constraints
- Review and enforce with staff the need for consistent, clear, and accurate documentation in the medical record to support outcomes
- Update team and medical staff with new regulations related to assessments and reimbursement
- Graduate of an accredited School of Nursing or a licensed Physical, Occupational, or Speech Therapist, PTA, COTA
- At least 2 years of Acute Inpatient Rehabilitation experience
- Computer knowledge
- Licensed or eligible for licensure in the state of Virginia by the State Board of Nursing or Rehab Licensing Board
