Job Title: Community Health Worker Lead

University of New Mexico is seeking a highly skilled Community Health Worker Lead to join the Office for Community Health Adult Inpatient Social Determinants of Health Team.

Job Summary:

The successful candidate will be responsible for supervising community health workers, creating training modules, and providing ongoing staff development instruction. They will also be responsible for coordinating cases with a multidisciplinary team to ensure patient Social Determinant of Health needs are being met during an inpatient hospital stay.

Responsibilities:
  • Supervise community health workers, including work allocation, training, and problem resolution.
  • Create training modules and continuing education curriculum for Community Health Workers.
  • Train new Community Health Workers and provide ongoing staff development instruction.
  • Coordinate cases with a multidisciplinary team to ensure patient Social Determinant of Health needs are being met during an inpatient hospital stay.
  • Perform administrative functions required to ensure efficient program operation.
Requirements:
  • High school diploma or GED.
  • At least 5 years of experience directly related to the duties and responsibilities specified.
  • Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
Preferred Qualifications:
  • Experience working with clients living with special needs.
  • Experience in providing comprehensive case management.
  • Experience in organizing, understanding, and connecting clients to resources.
  • Experience providing Community Health Worker services to medically and socially complex populations.
  • Experience working with the LGBTQ+ community.
  • Experience working in a multi-disciplinary setting.
