Job Summary

Albuquerque Health Care for the Homeless is seeking a Community Support Specialist to join our team. The ideal candidate will have a passion for connecting others with resources and providing support in a client-centered, trauma-informed, and harm reduction manner.

Key Responsibilities
  • Assess individual needs of clients and create individualized service plans with direct input from the client.
  • Work in an integrated manner with other AHCH programs to optimize client care.
  • Refer clients to appropriate programs and agencies for coordination of wrap-around services in the community.
  • Participate in client chart audits for peer review.
  • Provide direct service to clients with 60% of contacts conducted in home visits/community visits.
  • Perform duties in a trauma-informed and client-centered manner.
  • Ensure accurate and appropriate documentation of all client contacts in Electronic Health Record (EHR) within timeframe required by AHCH procedure and billing standards.
  • Attend program-specific meetings and trainings, including clinical supervision with Behavioral Health Clinical Director.
  • Attend organizational meetings and trainings.
Requirements
  • Ability to work in a client-centered, trauma-informed, and harm reduction manner to assist clients in meeting their goals.
  • Strong understanding of HIPAA and its application in case work.
  • BA in human services field or equivalent experience.
  • Two years working with individuals with significant or chronic behavioral/mental health issues, those experiencing homelessness, and/or those with substance use disorder.
  • Valid NM Driver's license and clean driving record.
Preferred Qualifications
  • CCSS certification.
  • Experience in trauma-informed care and harm reduction models.
  • Bilingual or Spanish speaking.
  • Past experience working with individuals with severe mental illness/chronic mental illness.
