Physician, Post Acute and Long Term Care Specialist

At Five Star Physician Services, we are committed to providing high-quality patient care in post-acute and long-term care settings. We are seeking a skilled Internal Medicine Physician to join our team of dedicated medical professionals.
About the RoleWe are looking for a physician who is passionate about delivering exceptional patient care and is committed to working in a team environment. As a member of our team, you will have the opportunity to work with a collaborative team of Medical Directors, Attending Physicians, and Nurse Practitioners to provide comprehensive care to our patients.
Responsibilities- Admit, round on, and discharge short-term rehab and long-term care patients at our partner Skilled Nursing Facilities
- Perform history and physical exams and medical rounds
- Provide cross coverage as needed by colleague medical staff
- Evaluate patients with changes in condition and attend to patients as requested by facility staff
- Collaborate with Nurse Practitioners and maintain compliance with all facility and Five Star Physician quality initiatives
- No after-hours or weekend On Call requirement
