Position Overview:
We are actively seeking a full-time Nurse Practitioner specializing in Post-Acute and Long-Term Care to serve our Nursing and Rehabilitation Centers. This role is pivotal in enhancing the quality of care provided to our patients.

Key Responsibilities:
  • Conduct comprehensive history and physical examinations.
  • Admit, round on, and discharge patients in collaboration with the medical team.
  • Provide evaluations for patients experiencing changes in their condition.
  • Ensure compliance with quality initiatives set forth by Five Star Physician Services.
  • Work closely with nursing staff to address patient needs and concerns.
