About the Role:
We are actively seeking a full-time Nurse Practitioner specializing in Post-Acute and Long-Term Care to provide essential services at our partner Nursing and Rehabilitation Centers.
Key Offerings:

Responsibilities:
As a vital member of a collaborative team, you will:
- Conduct admissions, rounds, and discharges for short-term rehabilitation and long-term care patients.
- Perform thorough history and physical examinations and manage medical rounds.
- Provide cross-coverage as necessary, addressing patient evaluations for changes in condition.
- Work closely with Nurse Practitioners to ensure compliance with all quality initiatives.
