UCHealth is seeking a skilled Behavioral Health Clinical Therapist to join our team at UCHealth Poudre Valley Hospital Main Hosp in Fort Collins, CO.
Job Summary:
We are looking for a highly motivated and compassionate Behavioral Health Clinical Therapist to provide top-notch care to our patients. As a Behavioral Health Clinical Therapist, you will work closely with our multidisciplinary team to deliver exceptional outcomes and make a meaningful difference in the lives of our patients.
Responsibilities:
- Implement counseling/therapy for assigned patients in individual and group settings.
- Interview and assess patients and/or their family members, caregivers, and/or legal representatives.
- Determine, provide, and/or arrange for needed internal and external services/interventions.
- Participate in case reviews/care conferences to evaluate progress.
- Assist with discharge planning and processes, including identification of community-based resources, and provide referrals as part of the discharge plan.
- Collaborate and coordinate patient's discharge planning needs and readiness for discharge with other members of the care team.
- Utilize appropriate teaching methods and/or referrals to ensure patient/family educational needs are met.
- Participate in training new staff, students, and interns as assigned.
Requirements:
- Master's Degree in Social Work, Counseling, Psychology, or a related field.
- State licensure as Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Marriage and Family Therapist (MFT), or Licensed Addiction Counselor (ACD), or other licensure/certification may be acceptable as approved by department leadership.
