About the Role:We are seeking a skilled Behavioral Health Clinical Therapist to provide top-notch care to patients in a direct patient care environment. As a member of our multidisciplinary team, you will implement counseling/therapy for assigned patients in both individual and group settings.
Key Responsibilities:- Interview and assess patients and/or patient's family, caregivers, and/or legal representatives.
- Determine, provide, and/or arrange for needed internal and external services/interventions.
- Participate and/or arrange case reviews/care conferences to evaluate progress.
- Assist with discharge planning and processes, including identification of community-based resources, and provide referrals as part of the discharge plan.
- Assess, collaborate, and coordinate patient's discharge planning needs and readiness for discharge with other members of the care team.
- Utilize appropriate teaching methods and/or referrals to ensure patient/family educational needs are met.
