Job Summary:

The Case Manager position at CHRIS 180 is a critical role in supporting the rehabilitation, environmental support, guidance, and resources coordination for youth and young adults. As a key member of our team, you will play a vital role in assisting individuals in gaining access to necessary services and creating environments that promote resiliency and support emotional and functional growth and development.

Key Responsibilities:

  • Assist with youth referrals from various sources, including Atlanta Public Schools, Fulton County Schools, Department of Juvenile Justice, Probation Office, Atlanta Police Departments, and Fulton County Courts.
  • Display positive role modeling, promote self-esteem, and reinforce socially acceptable behaviors.
  • Teach and reinforce professionalism in work tasks and employment, assess members' history of trauma, strengths, and needs, and develop and monitor individualized service plans.
  • Coordinate services and collaboration with other programs in the CHRIS 180 continuum, including transportation, team meetings, and communication with other providers and entities.
  • Manage caseload, make necessary partnerships to meet members' health, wellbeing, education, mental health, and employment needs, and research and attend trainings to educate self on best practices.
  • Employ tools to maintain compliance with governing agencies, manage budget, and oversee member progress and program participation.
  • Facilitate skill building, documentation requirements, and performance and conduct expectations, including maintaining confidentiality, professionalism, and customer service.
Requirements:

  • Bachelor's Degree in social services or a related field.
  • Experience working with youth and transition age young adults with mental health and/or substance abuse diagnoses.
  • Ability to effectively relate to consumers, parents/custodians, and staff of all social, economic, ethnic, and educational backgrounds.
  • Dependability, emotional stability, and physical capability to perform essential job functions.
Preferred Qualifications:

  • Technical expertise and ability to use available tools and resources to execute job responsibilities.
  • Accountability for work being completed accurately and in a timely manner.
  • Ability to collaborate with other staff to ensure best practices and sound decisions.
  • Ability to communicate effectively and adapt to changing situations.
Transportation:

This role may require providing transportation services for clients, ensuring safe and punctual transportation to and from programming. This includes responsibly driving clients while adhering to agency driving standards, maintaining a clean driving record, and prioritizing client safety and comfort during transit.

Schedule:

This is a full-time position with work occurring Monday through Friday, and occasional availability on two Saturdays per month. Workdays typically consist of 8 hours per day, with scheduled start and end times varying based on school or summer hours to accommodate the needs of school-age clients.
