Clinical Services Coordinator III

The Clinical Services Coordinator engages in intricate consultative functions and provides technical support.

This role encompasses the planning, development, and execution of agency programs while offering consultative guidance and technical assistance to program personnel, governmental bodies, community organizations, or the general populace.

This position may involve mentoring others.

Operates under general oversight, allowing for moderate discretion in the application of initiative and independent judgment.

Key Responsibilities:

The primary duties for this role include intervening at the child and family level, utilizing a multidisciplinary team approach that incorporates primary stakeholders involved with the child, including a STAR Health Medical Case Manager and professionals from various sectors such as legal, educational, juvenile justice, mental health, family members, and others. The aim is to expedite the transition of children from unlicensed care, avert entry into unlicensed care, and reduce recidivism.

Develop trauma-informed meetings with the interdisciplinary team and formulate trauma-informed, personalized care plans in collaboration with youth and families.

Collaborate with each child's caseworker to ensure that youth and their families are sufficiently prepared and supported to actively participate in these meetings.

The Clinical Services Coordinator should adhere to core principles and values to optimize family-based care, ensuring an individualized approach that avoids standardized step-down pathways.

Work alongside the caseworker and regional lead to guarantee that every child in an unlicensed setting receives the essential therapeutic and educational resources required.

The Clinical Services Coordinator will maintain contact with caseworkers for a minimum of 90 days post-discharge from unlicensed care to ensure that adequate services and supports are established for stabilization.

As children transition into licensed placements, the Clinical Services Coordinators will assist in facilitating discharge meetings and transitions for foster children in acute, subacute, and QRTP settings.

The coordinator will maintain consistent communication with all members of the child's support team.

The objective is to successfully transition children from one placement to another by supporting the caseworker and child while leading the team throughout the discharge process.

This position will collaborate closely with various entities to provide optimal support for children in DFPS care. The coordinator will also be responsible for offering discharge support to other DFPS field personnel.

May propose and coordinate initiatives to enhance program effectiveness. May contribute to the development and/or revision of policy and procedure manuals. May provide guidance to others.

Performs related duties as assigned.

Required Knowledge, Skills, and Abilities:
Familiarity with local, state, and federal regulations pertinent to the program area; public administration and management methodologies; statistical analysis techniques; budgeting processes; research methodologies; training and marketing strategies; and program management processes and techniques.
Knowledge of evidence-based clinical practices related to adolescent behavioral health. Understanding of behavioral variations associated with adolescent psychological disorders.
Proficiency in techniques proven to diagnose and treat adolescent psychological disorders. Knowledge of theoretical treatment alternatives for adolescent psychological disorders.
Awareness of preventive health care methodologies for adolescent psychological disorders.

Ability to remain cognizant of the reactions of others and comprehend the reasons behind those reactions.

Understanding of trauma-informed care and its implications for individuals and the system as a whole.

Knowledge of effective written and verbal communication techniques.
Proficiency in various computer applications necessary for data analysis.

Skill in identifying performance measures or indicators and utilizing a computer and relevant software.

Ability to gather, assemble, correlate, and analyze information; devise solutions to challenges; market programs; prepare reports; develop, evaluate, and interpret policies and procedures; communicate effectively; and provide guidance to others.
