Key Responsibilities:

  • Meet with administration, medical, and nursing staff to plan social services
  • Advocate for residents' rights and safety
  • Report abuse, neglect, or exploitation
  • Maintain professional relationships with facility staff and community resources
  • Complete social service histories and evaluations
  • Develop and implement care plans
  • Facilitate resident room changes and transitions
  • Attend resident council meetings and provide support
  • Perform quarterly reviews of resident charts

Requirements:

  • Bachelor's degree in Social Work or a related field
  • Current Social Worker license
  • Excellent communication and organizational skills
  • Ability to work in a fast-paced environment

Work Schedule: Monday through Friday, 8am-5pm

Work Environment: General Office
