Clinical Care Coordinator RN/Transitions Specialist

Job Summary:

As a Clinical Care Coordinator RN/Transitions Specialist at UnitedHealthcare, you will play a vital role in simplifying the healthcare experience for millions of people. You will work closely with patients, families, and healthcare teams to assess, plan, and implement care strategies that are individualized and directed toward the most appropriate level of care.

Key Responsibilities:

  • Assess and identify patient needs, developing personalized care plans that address their unique requirements.
  • Collaborate with healthcare teams to ensure seamless transitions and continuity of care.
  • Communicate effectively with patients, families, and healthcare providers to ensure quality, coordinated care.
  • Advocate for patients and families, ensuring their needs and choices are represented and supported by the healthcare team.

Requirements:

  • Current RN unrestricted license in the state of Texas.
  • 2+ years of experience working within the community health setting or in a healthcare role.
  • Familiarity with Microsoft Office, including Word, Excel, and Outlook.
  • Reliable transportation and ability to travel in assigned region to visit Medicaid members in their homes and/or other settings.

Preferred Qualifications:

  • 1+ years of experience with long-term care services and support, Medicaid or Medicare.
  • Ability to create, edit, save, and send documents, spreadsheets, and emails.
  • Knowledge of the principles of most integrated settings, including federal and state requirements.
