Position Overview

Are you passionate about connecting individuals with essential resources? Do you have experience working with vulnerable populations, including the elderly and those with disabilities? If so, consider becoming a part of our expanding team as a Housing Support Coordinator.

In this role, you will be responsible for facilitating access to community resources for tenants with disabilities residing in HUD-supported housing. This position offers a full-time schedule with flexible hours, allowing you to make a meaningful difference in the lives of those you serve.

Key Responsibilities
  • Provide informal case management, including intake and referral services to tenants requiring assistance.
  • Conduct assessments of health, psychological, and social needs, and develop tailored service plans.
  • Establish connections with local agencies and service providers to ensure the best possible support for tenants.
  • Create and maintain a directory of community resources for staff and tenants.
  • Educate tenants about available services, application processes, and their rights, while advocating on their behalf.
  • Monitor the ongoing provision of services and keep relevant parties informed of tenant progress.
  • Facilitate volunteer support programs and help tenants build informal support networks.
  • Conduct training sessions for tenants on tenancy obligations and other relevant topics.
  • Collaborate with team members to enhance understanding of aging in place and service coordination.
  • Present workshops or informational sessions on topics of interest to tenants.
Qualifications

To excel in this role, candidates should possess the following:

  • A Bachelor's degree in social work, sociology, psychology, or a related field, or an equivalent combination of education and experience.
  • A minimum of three years of relevant experience, particularly with older adults or individuals with disabilities.
  • Knowledge of supportive services and resources available for these populations is preferred.
  • A valid driver's license and reliable transportation to fulfill job responsibilities.
