Position Overview

La Frontera Center Inc. is seeking a committed professional to deliver vital support services to children and families facing challenges. In this role, you will be instrumental in crafting, assessing, and managing treatment strategies and recovery initiatives.

Main Responsibilities:
  • Build respectful and outcome-oriented relationships with clients receiving assistance
  • Create thorough clinical evaluations and service plans incorporating feedback from the child, family, or recovery team
  • Oversee and execute service plans in accordance with State guidelines
  • Keep precise clinical records and document progress accurately
  • Provide services through diverse interactions and support systems
  • Evaluate clients' responses to services and collaborate on necessary adjustments
  • Lead meetings to review progress and address needs
  • Work with external organizations for additional resources
  • Ensure transportation arrangements to essential services
  • Record all interactions and services rendered
Requirements:
  • Bachelor's or master's degree in a relevant field, or equivalent professional experience
  • Understanding of Managed Care frameworks and provider networks
  • Experience in case management and mental health programs
  • Valid driver's license and dependable transportation
  • Bilingual (Spanish/English) skills are preferred
