Job Summary

DePaul is seeking a highly skilled Intake Specialist to join our team in Depew, NY. As an Intake Specialist, you will play a critical role in processing referrals for potential placement in our licensed housing programs.

Key Responsibilities
  • Assist the Director in generating agency referrals through public contacts, presentations, mailings, and community involvement.
  • Conduct and/or coordinate designated referrals for pre-intake screenings, tours, and dinner visits.
  • Arrange and/or provide transportation to and from site visits during the screening and/or move-in process.
  • Review and gather all necessary referral material and document this information into the database.
  • Serve as an agency representative on various Single Point of Entry County meetings as instructed by the Director.
  • Attend and participate in weekly Risk Management meetings to review and discuss potential referrals and stay aware of individuals' housing plans.
  • Stay knowledgeable of all agency programs, their admission criteria, and any applicable changes.
  • Utilize person-centered practices and information gathered throughout the assessment process to make recommendations for placement.
  • Respect and maintain resident confidentiality and demonstrate a caring and positive attitude toward all residents, family members, staff, and service providers.
  • Work closely with the Director to monitor the status of referrals, the waiting list, and any potential resident movement within the agency.
  • Serve as a representative on various committees both within the agency and outside the agency to promote teamwork and community involvement.
  • Work with the Director to ensure that a disposition in writing is submitted for all referral applications within the required time frame.
Requirements
  • Bachelor's degree in Psychology or Human Services field required, with one year of experience working with individuals with psychiatric disabilities (preferably in a residential setting) preferred or an Associate's degree in Human Services field with two years of experience working with individuals with psychiatric disabilities (preferably in a residential setting).
  • A clean NYS driver's license is required, and the use of one's personal vehicle is also required.
