Intake Coordinator

We are seeking a highly skilled Intake Specialist to join our team at The DePaul Group. As an Intake Specialist, you will be responsible for processing referrals for potential placement in our licensed housing programs. This includes all necessary steps from pre-placement assessment to admission into the program.

• Assist the Director with generating agency referrals through public contacts, presentations, mailings, and community involvement.
• Conduct and/or coordinate designated referrals for pre-intake screenings, tours, and dinner visits.
• Arrange and/or provide transportation to and from site visits during the screening and/or move-in process.
• Review and gather all necessary referral material and document this information into the database.
• Represent the agency at Single Point of Entry County meetings as instructed by the Director.
• Participate in weekly Risk Management meetings to review and discuss potential referrals and housing plans.
• Stay knowledgeable of agency programs, admission criteria, and any applicable changes.
• Utilize person-centered practices to make recommendations for placement.
• Maintain resident confidentiality and demonstrate a caring attitude toward all residents, family members, staff, and service providers.
• Work closely with the Director to monitor referral status, waiting lists, and resident movement.
• Represent the agency on various committees to promote teamwork and community involvement.
• Ensure timely submission of dispositions for all referral applications.

A Bachelor's degree in Psychology or a Human Services field is required, with one year of experience working with individuals with psychiatric disabilities preferred. An Associate's degree in Human Services with two years of experience is also acceptable. A clean NYS driver's license and personal vehicle are required.
