Transition Support Specialist

Oswego County Opportunities (OCO) is seeking a Transition Support staff person to join our team. As a Transition Support staff, you will perform a variety of direct care, household, administrative, and recreational tasks to facilitate the smooth daily operation of the assigned Individualized Residential Alternative (IRA) program sites and ensure the safety and well-being of individuals during program changes.
Key Responsibilities- Provide person-centered support to individuals and adhere to OPWDD core competency expectations for a DSP.
- Follow individual protective oversight plans to ensure safety and protection of individuals and respect their rights.
- Assist with daily living routines, including hygiene practices, toileting, and grooming, and prepare individuals for daily activities and evening routines.
- Administer and/or assist with medications in accordance with OPWDD regulations and Agency Procedures, supervise individual self-medication, and document accurately.
- Assist with medical needs of individuals, participate in appointments, and complete necessary documentation.
- Plan menus, grocery shop, and prepare meals to meet consumers' dietary needs.
- Perform counseling and coaching of consumers, analyze situations, and use good judgment and proper intervention techniques.
- Participate in and implement habilitation plans, ensure provision of Medicaid billable services with accurate documentation and billing records using Precision Care software, and report concerns to supervisor.
- Ensure proper handling of Agency money and documentation for approved purchases, assist with individuals' personal allowance purchases, and ensure correct accounting and balances with documentation according to Agency and program procedures.
- Assist with house cleaning, laundry, and routine household maintenance according to assignment and schedule per site requirements to ensure safety and a sanitary environment.
- Schedule and participate in individual activities, ensure community integration, and provide transportation using agency van when available or personal vehicle if needed.
- Document and report incidents in accordance with OPWDD regulations and Agency procedures.
- Assist consumers as needed during an overnight shift.
- Follow Agency policy and procedures in emergency and non-emergency situations, handle situations that arise, and seek assistance on unusual situations and report concerns to supervisor or on-call manager.
- Provide first-aid treatment as necessary.
- Report concerns that arise during assigned shift to supervisor.
- Maintain paperwork as required by state and program regulations and Agency policy in an accurate and timely manner.
- Maintain confidentiality of individual, program, and site-related information.
- Must be adaptable and have the ability to work assigned schedule and be flexible with work schedule to meet program needs.
- Must be able to relate well with developmentally disabled individuals and their families.
- Must be able to exercise good judgment in carrying out duties, follow regulations, and defuse behavioral concerns.
- Must have the ability to work with others in a warm, professional manner and be a positive role model to individuals.
- Must maintain professionalism at work and when representing the Agency.
- Must have acceptable physical ability to carry out the position, including the ability to perform within the DD program Functional Job Description, including but not limited to: lifting regularly, pushing wheelchairs, assisting in transferring individuals, and climbing stairs.
- Must maintain all required certifications and trainings and attend additional trainings as required for skill enhancement.
- High School Diploma or equivalency preferred.
- 1-3 year's DSP experience preferred.
- Any equivalent combination of education, experience, and training.
