Job Title: Transition Support Specialist

Oswego County Opportunities (OCO) is seeking a dedicated and compassionate Transition Support Specialist to join our team. As a key member of our residential program, you will provide person-centered support to individuals with developmental disabilities, ensuring their safety, well-being, and empowerment.

Job Summary:

The Transition Support Specialist will perform a variety of direct care, household, administrative, and recreational tasks to facilitate the smooth daily operation of our Individualized Residential Alternative (IRA) program sites. This role requires excellent communication and interpersonal skills, as well as the ability to work collaboratively with individuals, families, and other stakeholders.

Key Responsibilities:
  • Provide person-centered support to individuals with developmental disabilities, adhering to OPWDD core competency expectations.
  • Assist with daily living routines, including hygiene practices, toileting, and grooming.
  • Administer and/or assist with medications, documenting accurately and sharing concerns with supervisory staff.
  • Participate in medical appointments, complete necessary documentation, and ensure provision of Medicaid billable services.
  • Assist with meal preparation, planning, and grocery shopping, meeting consumers' dietary needs.
  • Perform counseling and coaching, analyzing situations and using good judgment and proper intervention techniques.
  • Implement habilitation plans, ensuring provision of Medicaid billable services and accurate documentation.
  • Ensure proper handling of Agency money and documentation for approved purchases.
  • Assist with house cleaning, laundry, and routine household maintenance.
  • Schedule and participate in individual activities, ensuring community integration and providing transportation.
  • Document and report incidents, maintaining confidentiality of individual, program, and site-related information.
Requirements:
  • High School Diploma or equivalency preferred.
  • 1-3 years of DSP experience preferred.
  • Ability to work assigned schedule and be flexible with work schedule to meet program needs.
  • Excellent communication and interpersonal skills.
  • Ability to exercise good judgment and follow regulations.
  • Physical ability to perform duties, including lifting, pushing wheelchairs, and assisting with transfers.
