About the RoleThis is a unique opportunity to join our team of compassionate professionals who are passionate about making a difference in the lives of others. As a Clinical Care Specialist, you'll be responsible for:
- Conducting thorough assessments of patient needs and developing personalized care plans
- Coordinating with healthcare teams to ensure seamless care delivery
- Providing education and support to patients and their families

- Collaborating with colleagues to achieve exceptional patient outcomes
To be successful in this role, you'll need:
- A Bachelor of Science degree in Nursing (BSN) or equivalent experience
- At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role
- Current CPR certification
- Excellent communication and interpersonal skills
- Knowledge of state and federal home health agency regulations and compliance standards
