Position Summary
This role is responsible for overseeing the clinical services delivered in accordance with established standards of patient care as dictated by state regulations, accreditation bodies, and organizational policies. The coordinator ensures effective resource management to address patient needs while achieving the financial, operational, and quality objectives of the branch. Collaboration with leadership is essential to enhance care and services across the organization, ensuring operational and financial efficiency.Key Responsibilities
- Provide administrative and clinical support, acting as a liaison between physician practices, hospital case management, and other referral sources to meet patient and staff requirements.
- Analyze data to prioritize complex situations, facilitating appropriate actions based on the urgency of patient health, safety, and service delivery.
- Identify and address barriers to resolution of situations or complaints, collaborating with necessary resources or leadership for timely intervention.
- Review and assist in the creation of patient-specific clinical care plans, ensuring effective communication among all parties involved to support quality outcomes and patient satisfaction.
- Support clinical staff in executing the established care plan, coordinating service delivery, and implementing strategies to promote health and safety.
- Evaluate progress towards desired outcomes through documentation review, interdisciplinary meetings, and case conferences.
- Ensure compliance with billing practices by maintaining up-to-date knowledge of payer requirements and regulations, completing workflows promptly, and identifying opportunities for reimbursement.
- Delegate care responsibilities to appropriate healthcare personnel in line with legal and policy guidelines.
- Engage with patients, families, and other stakeholders in the clinical practice process.
- Utilize available resources to plan and deliver clinical services that are safe, effective, and financially sound.
- Participate in committees, shared governance, or other decision-making activities within the facility.
The primary work environment is an office setting, requiring extended periods of sitting and computer use. Duties may involve walking, standing, lifting, reaching, and other physical activities. The role necessitates the ability to lift and support weights up to 35 pounds, manage medical equipment, and assist patients in various settings. Proficiency in English communication is essential, along with intact sensory functions and fine motor skills. Critical thinking and the ability to respond swiftly to changes in patient conditions are vital.
Qualifications
Candidates must be graduates of an accredited nursing program, with a BSN preferred. Basic Life Support (BLS) certification is required as per policy. A minimum of one year of experience in a related field is necessary. A current RN license or temporary license in the state of practice is mandatory, with additional education or certifications as required by the nursing leadership.
