Job Summary

The Social Worker will play a critical role in our team, working closely with patients, families, and healthcare providers to ensure that our patients receive the best possible care. This role requires a strong understanding of social determinants of health, community resources, and patient advocacy.

Key Responsibilities
  • Develop and implement individualized care plans that address the unique needs of each patient
  • Provide education and support to patients and families on health-related topics, including disease management, medication adherence, and healthy lifestyle choices
  • Connect patients with community resources and services that can help them address social determinants of health, such as food insecurity, housing instability, and transportation barriers
  • Collaborate with healthcare providers to ensure that patients receive comprehensive, coordinated care
  • Participate in interdisciplinary team meetings to discuss patient cases and develop care plans
Requirements
  • Master's degree in Social Work (MSW) or Bachelor's degree in Social Work (BSW)
  • 2+ years of experience in a healthcare setting, preferably in a social work or patient advocacy role
  • Strong understanding of social determinants of health and community resources
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
