Position Overview:

Key Responsibilities:

  • Act as a positive and effective representative of The Center, introducing the complete range of services to hospital case managers, physicians, nursing agencies, and other referral sources.
  • Ensure continuous updates and education for referral sources.
  • Oversee the successful clinical coordination of care from the referral source into the home environment on behalf of The Center.
  • Utilize clinical expertise to assist referral sources and/or patients with:
    • identifying suitable candidates for home infusion and/or home health care
    • assessing the ability of patients and/or caregivers to provide care
    • educating and training patients regarding therapy and supplies, while clarifying care expectations in the home setting
    • facilitating medication connections at the hospital/facility to expedite discharges into home settings, as appropriate
  • Develop, maintain, and enhance core therapy and service line admissions to The Center through strong referral source relationships, providing excellent customer service to assigned target accounts.
  • Identify opportunities for expanding business with current referral sources/business associates and involve the Director of Sales & Marketing or the General Manager as necessary.
  • Demonstrate comprehensive knowledge of core therapy referrals and qualifying criteria by payor source, including TPN, Enteral Nutrition, and Inotropic Agents for Medicare.
  • Assess current and future clinical and service needs for existing and new accounts.
  • Coordinate and/or conduct in-services and continuing education programs for referral sources and their support staff as needed.
  • Collaborate with referral sources (e.g., hospital case managers, home health agencies, physician offices) and The Center staff to facilitate care coordination for new patient referrals.
  • Manage and communicate all discharge planning activities for new patient referrals with The Center personnel and referral sources, including:
    • Intake
    • Clinical Operations: Pharmacy and/or Nursing
    • Warehouse Operations, if necessary
    • Reimbursement, if applicable
  • Document coordination of care activities accurately, including:
    • home infusion and/or nursing services utilized
    • all communications with patients, referral sources, physicians, and Center personnel
    • all patient and/or caregiver education and training
  • Ensure documentation is legible, accurate, complete, and compliant with company policies and procedures.
  • Maintain up-to-date records for referral sources and any other requested documentation in line with The Center's goals.
  • Assist in activities aimed at achieving favorable net revenue and gross profit targets for The Center.
  • Represent The Center in a professional and positive manner.
  • Participate in relevant office meetings, sales meetings, training programs, and regional and national trade shows and conventions as directed by the Director of Sales & Marketing or the General Manager.
  • Coordinate and cross-train departmental duties to maximize productivity and efficiency.
  • Stay informed about developments in the field of infusion therapy.
  • Adhere to all company policies and directives to fulfill organizational objectives.
  • Communicate any issues directly to the Director of Sales & Marketing or the General Manager.
  • Perform additional duties as assigned by management.
Qualifications:
  • Registered Nurse (RN) or Licensed Practical Nurse (LPN)
  • Diploma, Associate's, or Bachelor's degree from an accredited nursing program.
  • Minimum of 3 years of IV-related experience.
  • At least 2 years of home health care experience.
  • 1 year of case management experience.
  • Experience in sales and customer service.
  • Current RN licensure in the state(s) where The Center operates; additional licensure may be required in multi-state service areas.
  • Valid driver's license and auto insurance.
  • Current CPR certification.
  • CRNI certification preferred.
  • Professional appearance and adherence to company dress code.
  • Excellent customer service skills (both verbal and written) and effective interpersonal skills for positive interactions with diverse personality types.
  • Proficient computer skills.
Physical Requirements:
  • Regular driving required; may work in outdoor weather conditions while traveling.
  • Frequent use of hands for demonstrating medication administration, equipment use, and administering medications.
  • Regularly required to stand, walk, sit, and reach with hands and arms.
  • Occasionally required to lift and/or move up to 30 pounds.
