The Clinical Social Worker plays a crucial role in enhancing patient and family outcomes by addressing intricate psychosocial and economic challenges. Utilizing advanced practice skills, this position mobilizes essential resources to mitigate risks and acts as a vital link between the healthcare facility and the community.
Key Responsibilities- Deliver comprehensive social work services to patients and families, enabling them to effectively engage in their treatment and manage the realities of their health conditions.
- Coordinate transitions of care for patients with significant psychosocial needs, ensuring a seamless process.
- Maintain up-to-date knowledge of local, state, and federal resources that can provide education and support to patients and families.
- Collaborate with the Care Manager and interdisciplinary team members to review and discuss the multidisciplinary care plan, ensuring accurate documentation and communication.
- Participate in Multidisciplinary Rounds or Huddles, providing insights on psychosocial factors affecting patient care.
- Monitor and adjust the social work care plan based on the evolving needs and conditions of patients.
- Identify potential barriers to successful community reintegration and work towards resolving these issues through appropriate channels.
- Communicate effectively with all healthcare team members to facilitate timely and safe patient care, ensuring families receive necessary resources and education.
- Assess psychosocial risk factors by evaluating prior functioning levels and support systems, and provide counseling regarding the emotional, social, and financial impacts of health issues.
- Offer brief crisis intervention and therapeutic support, particularly for frequent users of healthcare services.
- Conduct follow-up calls post-discharge for high-needs patients to prevent readmissions.
- Facilitate complex discharge planning through psychosocial assessments and the development of short-term treatment plans.
- Lead family meetings to foster comfort and decision-making.
- Assist in obtaining financial resources, legal guardianship, and psychiatric referrals, while also addressing issues of abuse and neglect.
- Engage in Care Coordination Rounds to enhance collaborative care efforts.
- Contribute to team dynamics by resolving conflicts and promoting a unified approach to patient and family interactions.
- Lead or co-facilitate support and education groups for at-risk populations.
- Collaborate with community agencies to enhance service responsiveness and effectiveness.
- Actively participate in clinical performance improvement initiatives.
- Ensure all processes are completed to facilitate patient transitions through various levels of care.
- Provide specialized mental health services, including crisis intervention and addiction assessments, while employing de-escalation techniques as necessary.
Applicants should possess a Master of Social Work (MSW) degree from a C.S.W.E.-accredited institution. A minimum of two years of experience in a healthcare setting is required. Candidates must obtain Registered Clinical Social Worker Intern (RCSWI) status within six months of employment, with a preference for Licensed Clinical Social Worker (LCSW) designation.
