Job Title: Care Coordination Specialist

Job Summary:

We are seeking a detail-oriented and compassionate Care Coordination Specialist to join our team at Emanuel Medical Ctr campus. As a key member of our care coordination team, you will play a vital role in ensuring the smooth transition of our acute care patients to the next level of care in their recovery.

Responsibilities:

  • Function as the primary point of contact between care management teams and post-discharge facilities/agencies.
  • Fax clinical information for placement at post-discharge facilities such as SNF, ICF, AFH, Dialysis chairs, and Home Health.
  • Schedule post-discharge PCP appointments and complete necessary documentation.
  • Establish PCP for patients without a PCP.
  • Arrange discharge transportation for patients and document in medical records.
  • Check Medicaid eligibility and inform staff accordingly.
  • Deliver Secondary IMM when directed by the Care Management Team.
  • Transcribe Utilization Review phone calls and communicate information to the Utilization Management RN.
  • Demonstrate effective interpersonal skills and communicate concerns to management.

Requirements:

  • Bachelor's degree preferred or equivalent experience.
  • Two years of experience in healthcare, with administrative program support function experience preferred.
  • Strong analytical and customer-support skills.
  • Ability to manage conflict, competing priorities, and problem-solve.
  • Strong verbal and written communication skills.
  • Medical terminology background preferred.
