Job SummaryThe Mental Health Professional II is a key member of our multidisciplinary team, responsible for delivering exceptional patient-focused care by providing supervised mental health services to patients and their families. Your responsibilities will include conducting assessments, offering psycho-education, leading process groups, developing treatment plans, and facilitating therapeutic counseling sessions using evidence-based interventions in one-on-one, family, and group therapy.
Key Responsibilities- Conduct one-on-one, group, and family psychoeducational and process groups
- Develop treatment plans and attend treatment planning meetings and reviews on an ongoing basis
- Assess environment and patient's safety status and report/respond to actual and potential safety issues or changes in the patient's condition
- Identify risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement)
- Complete assessments, group notes, and all other documentation promptly and in compliance with requirements, as appropriate to scope
- Collaborate with the multidisciplinary team to assess and coordinate patients' discharge planning needs and readiness for discharge
- Conduct post-discharge follow-up as indicated
To be successful in this role, you will need:
- Masters Degree
- Basic Cardiac Life Support, or CPR, or CPR for Healthcare Providers, or Heart Saver CPR must be obtained within 30 days of employment start date
- Licensed Clinical Social Worker (LCSW), or Licensed Family Therapist (LFT), or Licensed Professional Counselor (LPC), or Licensed Marriage and Family Counselor (LMFC), or Mental Health Counselor Intern, or Licensed Master Social Worker (LMSW)
