Position Overview: RN, Heart Transplant Coordinator
Duke University Health System is seeking a dedicated Registered Nurse to fulfill the role of Heart Transplant Coordinator, embracing our mission of Advancing Health Together.

The Heart Transplant Program at Duke University Hospital is a dynamic program, performing over 100 transplants annually while ensuring exceptional care and outcomes for each patient.

The Heart Transplant Coordinator will engage with heart transplant patients throughout all stages of their journey, collaborating with a multidisciplinary team that includes both adult and pediatric heart coordinators.

This role involves providing advanced nursing care for patients before and after heart transplantation, available 24/7, ensuring comprehensive support across the continuum of care.

Work Schedule: Monday - Friday, hours vary based on program needs. This is a full-time position requiring rotational weekends and holidays for on-call coverage.

Key Responsibilities:

  1. Deliver advanced nursing care for patients throughout the transplant process, ensuring continuous support.
  2. Coordinate evaluations for pre-transplant patients, optimizing the use of clinical resources.
  3. Conduct thorough assessments of transplant organ function and overall health, determining the need for further interventions.
  4. Facilitate scheduling for diagnostic studies and procedures, maintaining fiscal responsibility.
  5. Participate in developing and implementing care protocols in collaboration with the healthcare team.
  6. Support nursing staff in assessing, planning, implementing, and evaluating patient care.
  7. Monitor care processes and outcomes to identify areas for quality improvement.
  8. Provide on-call coverage, coordinating activities prior to transplant and serving as a medical triage resource.
  9. Manage the transplantation process when a donor is available, ensuring effective communication among all parties involved.
  10. Maintain accurate documentation in accordance with regulatory guidelines.
  11. Communicate promptly with team members and families regarding patient care.
  12. Assist in discharge planning for patients, coordinating necessary referrals and resources.

Education and Experience:
A Bachelor of Science in Nursing (BSN) is required; a Master of Science in Nursing (MSN) is preferred. Candidates should have a minimum of five years of experience as a bedside Registered Nurse, with at least two years in transplant nursing.
