Position: RN, Transplant Coordinator - Heart Program
Role Overview:
The Heart Transplant Program at Duke University Hospital is a vibrant initiative, performing over 100 transplants annually while ensuring exceptional care for each patient to guarantee outstanding experiences and outcomes.
The Heart Transplant Coordinator will engage with heart transplant patients throughout all stages of their journey. This role is integral to a multidisciplinary team that includes both adult and pediatric heart coordinators.
The coordinator will deliver advanced nursing care for patients pre- and post-heart transplantation, providing support across the continuum of care, available 24/7.
This is a salaried position requiring rotational weekends and holidays for on-call duties.
Key Responsibilities:
- Conduct comprehensive patient assessments in clinic settings, manage phone triage, and provide patient education to ensure high-quality care.
- Coordinate the solid organ heart transplantation process for both pediatric and adult recipients, overseeing follow-up care from pre-transplant through post-transplant stages.
- Collaborate with healthcare team members to enhance patient care, develop, support, and educate nursing staff.
- Make clinical decisions based on current research and best practices to address complex patient needs.
- Participate in the development and implementation of care protocols and standards in collaboration with the multidisciplinary team.
- Provide on-call coverage, coordinating activities prior to transplant and serving as medical triage for outpatients.
- Maintain accurate documentation in compliance with United Network of Organ Sharing (UNOS) guidelines.
- Educate patients and families about transplantation processes and provide resources for understanding pre- and post-transplant issues.
- Engage in research activities and disseminate findings to improve transplant care practices.
Qualifications:
Education: BSN required; MSN preferred.
Experience: Minimum of 5 years as a bedside Registered Nurse, with at least 2 years in transplant services.
Skills and Abilities:
Strong attention to detail, effective communication, and documentation skills are essential for collaboration with various healthcare professionals and patient families.
