Position Overview: RN, Heart Transplant Coordinator
This role involves working within the Heart Transplant Program at Duke University Hospital, which performs over 100 heart transplants annually, ensuring exceptional care and outcomes for each patient.

The Heart Transplant Coordinator will engage with heart transplant patients throughout all stages of their journey, collaborating with a multidisciplinary team that includes both adult and pediatric heart coordinators.

Key Responsibilities:

  1. Deliver advanced nursing care for patients pre- and post-heart transplantation, available 24/7.
  2. Coordinate the evaluation process for pre-transplant patients, optimizing the use of clinical resources.
  3. Conduct thorough assessments of transplant organ function and overall health, determining the need for further evaluation or intervention.
  4. Facilitate scheduling for diagnostic studies and procedures, ensuring fiscal responsibility.
  5. Participate in the development and implementation of care protocols for transplant patients in collaboration with the healthcare team.
  6. Provide on-call coverage, managing activities prior to transplant and serving as a medical triage resource for outpatients.
  7. Oversee the transplantation process when a donor is available, ensuring effective communication among all parties involved.
  8. Maintain accurate documentation in accordance with United Network of Organ Sharing (UNOS) guidelines.
  9. Educate patients and families about the transplant process and available resources.
  10. Contribute to research and quality improvement initiatives related to transplant care.

Qualifications:
A Bachelor of Science in Nursing (BSN) is required; a Master of Science in Nursing (MSN) is preferred. Candidates should have a minimum of five years of experience as a bedside Registered Nurse, with at least two years in transplant nursing.

Skills and Attributes:
Strong attention to detail, effective communication skills, and the ability to collaborate with various healthcare professionals are essential for success in this role.

This position is vital to the success of the Heart Transplant Program and contributes significantly to the overall mission of Duke Health.
