We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at Duke Health. As a Home Health Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities- Plan, provide, and document professional nursing care for patients in their homes, utilizing the nursing process and adhering to agency policies and regulations.
- Assess patient needs, develop individualized care plans, and implement interventions to promote optimal health outcomes.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
- Conduct comprehensive assessments, including OASIS evaluations, to determine patient reimbursement and publicly reported outcomes.
- Develop and implement effective discharge plans to ensure smooth transitions and optimal patient outcomes.
- Provide education and support to patients and families, promoting self-care and independence.
- Document care and services provided, ensuring accurate and timely recording of patient information.
- Participate in performance improvement initiatives, contributing to the development and implementation of quality improvement projects.
- Current RN license in NC or compact RN licensure from participating state.
- Current State of North Carolina Cardio-Pulmonary Resuscitation Certificate (CPR) (American Heart Association only).
- 1-2 years of med-surgical, critical care, or home health/hospice/infusion experience preferred.
- Valid driver's license and current automobile insurance coverage.
- Knowledge of Medicare and Medicaid regulations, including JCAHO and DFS standards, ICD-9 Coding.
- Ability to assess nursing needs of acute and chronically ill patients and their families.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
