Join Our Team as a Hospice RN Case Manager

We are seeking an experienced Registered Nurse (RN) to join our team as a Hospice RN Case Manager at Memorial White Rose Hospice in York, PA. As a key member of our interdisciplinary team, you will be responsible for delivering high-quality end-of-life care to patients and their families.

About the Role
  • Assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient, continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered.
  • Make initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
  • Identify patients' physical, psychosocial, and environmental needs, reassessing as needed, no less than every 15 days.
Key Responsibilities
  • Initiate and coordinate the plan of care while maintaining accurate and relevant clinical notes regarding patients' conditions.
  • Document problems, appropriate goals, interventions, and patients'/families' responses to hospice care.
  • Collaborate with patients/families, attending physicians, and other members of the Interdisciplinary Group (IDG) in providing daily patient and family care.
Requirements
  • One year of clinical experience as a Registered Nurse (RN).
  • Current RN licensure in the state of practice and one year of clinical experience.
  • Current CPR Certification.
  • Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
