Hospice Case Manager Professional

We’re seeking an experienced RN Case Manager who can take their nursing career to new heights as an end-of-life care expert. As a member of our team, you can expect:

As our ideal candidate, you will assume full nursing responsibility for delivering high-quality care to each hospice patient. Your key responsibilities will include:
- Making initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral
- Identifying patients’ physical, psychosocial, and environmental needs through regular assessments (at least every 15 days)
- Developing and implementing personalized plans of care while maintaining accurate clinical notes on patient progress
- Collaborating with patients, families, attending physicians, and IDG members to deliver seamless care
In this role, you’ll leverage your clinical experience and expertise to make a tangible impact on patients’ lives. We welcome candidates with:
- A minimum of one year of clinical experience
- Current RN licensure in the state of practice and one year of clinical experience
- Current CPR certification
- A valid driver’s license, vehicle insurance, and reliable transportation or access to public transportation
We offer a competitive salary range estimated at $65,000 - $80,000 per annum, commensurate with experience.
